Users can access all subdirectories of your main public directory, which you can assign anywhere you want (even CD0: if you have a CD-ROM drive.) Binary files can be automatically uuencoded before being sent out if you have Asher Feldman's UUxT uuencoder. Mailserver also allows you to define you own 'external' commands, which make it possible to run virtually any program or script by e-mail command. Mailserver has only be tested with AmiTCP 3.0b, but it should be possible to get it working with most mail systems quite easily.
